What happens when you call AccessOne?

  • Your call will be answered promptly, 24 hours a day, 7 days a week
  • You will speak with an experienced RN
  • You provide brief details on reason for call including service requested, patient demographics and summary of patient presentation
  • We will connect you via three-way call to an admitting physician or appropriate specialist
  • We will help coordinate air or ground transport
  • We will pre-admit your patient if appropriate
  • We will provide you with room assignment and phone number for nurse to nurse report
  • You will receive follow-up information on the patient for improved coordination of care


Information we need to expedite your call

Call AccessOne at (800) 433-2858. The AccessOne nurse will ask for the following information:

About You:

  • Facility/provider name
  • Provider call back number
  • Reason for call: tranfer, consult, direct admin, other
  • Service requested

About the Patient:

  • Patient’s legal name
  • Sex
  • Date of birth
  • Brief description of illness

Patient Transfer Process:

Please send the following items with the patient or transferring agency at the time of transfer:

  • Medical history and physical
  • Nursing record with current vital signs
  • Copies of all pertinent diagnostic studies
  • Laboratory results
  • Medication history
  • Signed Release of Information form
  • Information on advanced directives

To best facilitate continuity of care, please fax the following information to the number provided by your AccessOne nurse:

  • Face sheet
  • Copy of patient insurance card (both sides) and HMO authorization if non-emergent transfer
AccessOne can help coordinate air or ground transport as needed. Specialty team transports, such as neonatal or pediatric teams, are available with our transportation partner EAGLE III.
